How to Make a Claim Please tell us as soon as is practicable after the date of diagnosis or surgery
if you wish to make a claim. We will initially require:
- a fully completed claim form signed by the employer
- a fully completed personal statement, signed by the member. This
includes a consent that provides us with the authority to obtain
further information from their doctors as required under the Access
to Medical Reports Act
- where a spouse’s benefit is being claimed, an original
copy of the spouse’s marriage certificate.
Upon receipt of all our initial requirements, we will advise you
within 5 working days:
- of any further information we require to assess the validity of
the claim, or
- if we are unable to admit the claim and the reason(s) why.
We will then obtain details of the member’s medical history
and treatment for the present incapacity from their General Practitioner
and/or consultant.
