Group Critical Illness

A critical illness benefit is paid when an eligible scheme member suffers a condition or undergoes a procedure covered under the policy and survives a set period of time.

Please see our Guide to Conditions Covered for a comprehensive list of conditions that can be insured.

No payment will be made if we receive our required claim documentation later than two years from the date the illness was diagnosed or the procedure was undertaken.

Required information to make a claim

1. Claim Form – to be completed and signed by the employer.
2. Personal Statement – to be completed and signed by the employee or spouse/civil partner who is intending to claim. If the claim is on behalf of a child, then the employee must complete the form.
3. Medical Evidence – we encourage the claimant to send us supporting medical evidence. If this is not immediately available or any additional information is required, we will contact the claimant’s GP.

Once all documentation has been received, the claim will be assessed and we will inform the relevant persons of our decision.

Additional Documentation

If the claim is being made on behalf of the employee’s child or spouse, we will require extra documentation:

1. Spouse/civil partner – marriage certificate or civil partnership document.
2. Child – birth certificate or legal adoption certificate, if applicable.

Please note, we require original copies of all documents.

How long will it take to assess a claim?

We aim to confirm within five working days after we have received all the information we require:

  • we can admit the claim, or
  • what further information we require to assess the validity of the claim, or
  • the reason(s) why we are unable to accept the claim.
